Self-pay & private pay options

If you don’t have insurance or have reached your plan’s limit for physical therapy visits, you can still get the care and treatment you need. Mountain Land Physical Therapy Partners offers a self-pay fee for those who do not have health insurance or those who have maxed out their benefits.

Understanding your insurance benefits

Having insurance doesn’t always mean your care is free. Most plans ask you to share some of the cost. This can include things like deductibles, copays, and coinsurance. Understanding these terms before your visit can help you feel more prepared and avoid surprises.

Here’s a simple guide to the most common insurance words you may see.

TermWhat it means
DeductibleThe amount you must pay each year before your insurance starts helping pay for care. Once met, you will usually be responsible for a copay/coinsurance.
CopayA set fee you pay at each physical therapy visit.
CoinsuranceThe percent of the cost you pay after your deductible is met, you’re responsible for until your OOP is met.
PremiumThe monthly payment you make to keep your health insurance.
Out-of-pocket maximumThe most you should pay for covered care in a year. After that, insurance pays the rest.
In-network vs out-of-networkIn-network providers work with your insurance and usually cost less. Out-of-network providers may cost more.
ReferralA recommendation for physical therapy from another physician.
Prior authorizationPermission from your insurance company before treatment begins.
Visit limitsThe number of physical therapy visits your insurance will cover.
Date of serviceThe day you received treatment.
CPT codeA code that describes the service you received during your visit.
Billed amountThe full amount the clinic charges for a service.
Adjusted amountThe lower amount your insurance agrees to pay.
Patient responsibilityThe part of the bill you must pay.
Explanation of benefits (EOB)A statement from your insurance showing what was billed, what was paid, and what you may owe.
Medicare Part B coinsuranceWith Medicare Part B, you usually pay 20% of the approved cost for physical therapy after the deductible.

What to expect at your visit

When you come in, you will be prompted to pay at the time of service, this may be required if your insurance plan has a required copay. However, the final cost may change depending on your treatment needs and duration. It’s important to compare your receipt with your Explanation of Benefits (EOB) after your visit to ensure everything matches.

Questions to ask your insurance before starting physical therapy

Before your first visit, it helps to know the right questions to ask your insurance:

  • What is my deductible, and have I met it?
  • Does my deductible apply to physical therapy?
  • What is my copay or coinsurance for outpatient physical therapy?
  • Do I need a referral or prior approval?
  • How many physical therapy visits does my plan cover each year?
  • Is Mountain Land Physical Therapy Partners in-network with my insurance?
  • If I have Medicare, does my secondary plan cover the 20% coinsurance?

Pro tip: Sign up for your insurer’s online patient portal to check benefits, track deductible and view claims.
